Centered was created through a partnership between two community-based behavioral health centers, Jefferson Center for Mental Health and Mental Health Partners, and a clinically integrated network they formed called Front Range Health Partners.
Front Range Health Partners offers a variety of clinical and administrative support services for healthcare purchasers and employers working across the front range region of Colorado. This includes increasing access to a continuum of mental health and addiction recovery care, comprehensive support for individuals and families and proven performance and value.

Erin Knudson- Certified Health and Wellness Coach

Erin Knudson, MS, NBC-HWC

Erin has a Master of Science degree in behavioral therapy from Northwestern University. After practicing as a therapist for many years, she opened the door to a new career, receiving her training in Health and Wellness Coaching at the Mayo Clinic. She is actively practicing as a National Board-Certified Health and Wellness Coach. Her passion is optimizing the health and wellness of the human elements consisting of body, mind, and spirit. She focuses on identifying strengths and building Tiny Habits that keep her clients motivated to achieve their SMART Goals. Erin believes in integrating Western and Eastern Medicine to help a person heal and thrive optimally. She strives to help her clients learn to maximize the potential health of their human elements to thrive with higher levels of resiliency, functioning, and wellness. She brings meaning to her own life by being fully present with her loved ones, pets, friends, and new people. Erin engages in self-care by walking, hiking, engaging in yoga and meditative practices, experiencing spiritual rituals and traditions, reading about health and wellness, finding pleasure in things that taste, smell, look, sound, and feel wonderful, and laughing heartily at ridiculously silly things.

Raquel Schneider-Supervisory Consultant

Raquel Schneider, MA, LMFT

Raquel is a licensed marriage and family therapist practicing in Denver, CO. She received her Master of Arts in Marriage and Family Therapy from Regis University in 2016. Raquel has provided therapeutic services for children and families within intensive in-home, day treatment, residential treatment and outpatient programs. She enjoys implementing creative interventions that are tailored to a client’s needs, interests and strengths to help promote motivation, personal connection and growth. Raquel currently works as a Clinical Care Coordinator at Jefferson Center for Mental Health where she provides supervision to other clinicians at several pediatric locations. She is also providing therapeutic services at Sustaina Center for Women.

Kylie Slavek- Therapist

Kylie Slavek, MA, LPC, CRC

Kylie (she/her) is a Licensed professional counselor and mental health educator with a Master of Science in clinical mental health. She is passionate about helping clients build insight and develop skills to overcome challenges. Kylie artfully combines her love of Buddhist psychology with modern and holistic therapeutic approaches to help clients shift perspective and live with ease. Kylie specializes in working with anxiety, depression, chronic stress, and navigating career and life transitions. Her calm, open minded and accepting nature will allow you to embrace your true authenticity and support you in reaching your full potential. While Kylie’s clinical interests include trauma, breath work, somatic therapies, and career counseling, in her spare time she enjoys gardening, hiking, crafting, thrifting, yoga, and rock climbing.

Dustin Shivar
Dustin Shivar- Supervisory Consultant

Dustin Shivar, MA, LCSW

Dustin, is a Licensed Clinical Social Worker, licensed in North Carolina and Colorado, with over 10 years of experience serving adults, children/ adolescents, couples, and families in a variety of practice settings over my progressive career in human services. He attended East Carolina University, and he possesses a Master of Social Work degree from that institution earned in 2011. He has worked with patients struggling with an array of stressors including depression, anxiety, relationship impairments, family conflicts, bipolar disorder, OCD, and ADHD. Additional areas of expertise include: grief/loss, adjustment/ transition, building motivation, communication, relaxation, and improving self-concept.

Dustin describes his counseling style as “respectful, empathetic, interactive, engaging, direct, straight-forward, and practical while emphasizing the personal strengths, talents, wisdom, and self-awareness of the patient to succeed at using these strengths to resolve one’s own personal difficulties.” He believes in treating everyone with respect, sensitivity, and compassion, while not subscribing to established stereotypes. His therapeutic approach combines aspects of cognitive-behavioral, person-centered, solution-focused, and reality-based treatment modalities. He is skilled at tailoring dialog and treatment plans to meet each person’s unique and specific needs. In addition to providing therapy/counseling, he also brings 10+ years of experience providing clinical and administrative supervision in a variety of practice settings.

Kathryn Dunn- Supervisory Consultant

Kathryn Dunn, LSCW, LAC

Kaite, is a Licensed Clinical Social Work and Licensed Addiction Counselor focused on developing trauma-informed, culturally responsive, growth-oriented, and healing-centered services and systems. Working with children, adolescents, and adults in a variety of settings, including residential treatment, in-home services, primary care, and public schools. Experience developing and facilitating trainings and teaching as an adjunct social work faculty. Kaite currently works as a clinical coordinator on the integrated care services team at Jefferson Center for Mental Health.

Meghan Pataky
Meghan Pataky- Supervisory Consultant

Meghan Pataky, DSW, LCSW, CACII

Dr. Meghan Pataky is a licensed clinical social worker and addictions counselor practicing in Denver, CO. She began her career in social work as a residential counselor at a short-term treatment facility for children and adolescents with severe social, emotional, and behavioral disorders. Throughout her career, Meghan has continued to support the most at-risk children and families through work in child welfare, school-based services, private practice, and integrated medical care. Meghan has over 15 years of direct practice experience and over 12 years of supervisory and management experience. Meghan currently works as the Director of Integrated Care Services at Jefferson Center and as an Academic and Field Advisor for Columbia University’s School of Social Work. Meghan received her Masters of Science in Social Work from Columbia University in 2010 and her Doctorate in Social Welfare from New York University in 2018. Meghan’s academic and clinical experience position her to be an effective social work practitioner, supervisor, teacher, and manager.
